What to include in a delivery note or packing slip
These documents list what is in a shipment. They are not invoices. A packing slip often rides in the box so the receiver can check the contents. A delivery note is often signed when the goods arrive. Either way, the useful fields are the same: who shipped, who receives, the date, a reference to the order or PO, and quantities.
Backordered quantity matters. If the customer ordered 10 and you shipped 7, show 3 backordered instead of hoping they notice. Item numbers help the receiver match the slip to their PO. Prices are optional because many warehouses do not want dollar amounts on the dock.
How this fits with other documents
A buyer may send a purchase order. You ship against it and include this packing slip. Later you send an invoice for what shipped (and a later invoice for the backorder). If they pay on delivery, a receipt closes the loop.
This is not a bill of lading. A bill of lading is a carrier contract. Use your carrier’s BOL when you need one.
Who can use this
US wholesalers, e-commerce sellers who ship from a garage or 3PL, contractors sending materials to a job site, and offices forwarding equipment can print a slip that matches the box. Drivers can get a signature and a condition mark without carrying a full invoice.
US tips
Put the tracking number on the slip so customer service can find the shipment without opening the cart. Use the same item numbers as the PO. If something arrives damaged or short, the checkbox on the PDF is a starting record — follow your carrier’s claim process as well.

Packing the box vs proving delivery
Print two copies when it helps: one in the carton (packing slip, prices off) and one on the clipboard (delivery note, signature block). The receiver checks item numbers and quantities, then marks Good, Damaged, or Short. Photograph a damaged carton before you leave if you are the driver. That photo plus a signed note is easier to argue from than a memory of “it looked fine.”
US carriers (USPS, UPS, FedEx, regional freight) already give tracking. Putting that number on your slip does not replace their scan; it just ties your internal DN-1001 to their tracking so a customer-service search works. If you ship only a partial PO, say so in the notes and set backordered quantities. The next shipment can reuse the same order reference with a new delivery-note number.
Addresses should use street, city, state, and ZIP. Job sites often have a gate code or a receiving-hours note — put that in notes, not in the item grid. This page is for small-business shipments, not hazardous materials paperwork or export customs forms.
